We have been proud in providing Hospital discharge services since 2020 being the first VCSE sector organisation to have a contract for this service in southern Staffordshire. And over those six years have helped hundreds of people be discharged promptly to their home and support independent living.
Our role is to work alongside NHS Providers, and Community Hospitals to provide personalised care solutions.
We cover the service for Tamworth, Lichfield, and Burntwood areas and, after discharge we provide longer term support so don't just walk away.
Some of the things we do to work together to support this are:
- Fitting Key Safes
- Food Shopping
- Organising House Cleans or House Clearances
- Furniture Removal and Moving Furniture
- Welfare Calls
- Sign posting to community services.
According to The Health Foundation UK, most people admitted to hospital want to recover and return home as soon as possible. Extended stays in the hospital can lead to a loss of muscle strength by up to 5–10% within the first 7 days, which can hinder their return to living independently or increase the level of support they need.
Analysis by Age UK shows that the average excess bed day – the cost of a patient remaining in a hospital bed after their treatment has ended – costs £346
